Forms fields save to excel and 2 folders *BASIC BASIC* by kgethanllc

This project is extremely basic I just do not know how to use dreamweaver or basic html forms. I have a submission form that has 13 fields and a submission button. I need the fields to save to an excel sheet that is in the same folder of the html page... (Budget: $30-250, Jobs: CSS, Javascript, PHP, Ruby/Ruby on Rails, XML)
