Taxicab Fare Quote & Online Reservation Form by mzwayyed

This is a sub-project for a taxicab company website. I need to be able to offer a fare quote based on Starting Address and Ending Address (using google maps to calculate the driving distance), then the distance is plugged into a formula which will spit out the Fare Quote...
