Need someone to build PHP forms to MySQL from my mock-ups by iquotient

I am working on a remote database project. I need someone who can: 1. Take my Excel list of fields and tables 2. Create PHP front-end forms in PHP, Cold Fusion or Flex. 3. Create an environment that will allow one to enter data in the fields and store to MySQL server... (Budget: $30-250, Jobs: Cold Fusion, Flex, PHP, SQL)
