Word / PDF Conversion - FPDF / with a form by silverarm

I would like to take a 2 page word document and convert it to PDF. However we need to have fields within the pdf which are auto filled in, and a logo image embedded in the pdf file on the fly (top right)... (Budget: $30-250, Jobs: PHP)
